| I understand the Captain and crew are to provide safe voyage on the vessel MIJOY. The boat has been surveyed and verified to be in excellent condition, meeting all USCG safety requirements. It will be my responsibility to inform the captain and crew of any significant medical conditions and/or medications used that may be affected by vigorous exercise and/or boating. I will use the equipment provided "as if it were my own" and understand any "major" losses are my responsibility. I also agree that physical exercise and fishhooks potentially cause pain that cannot be deemed the responsibility of ... MIJOY ADVENTURES! ... and that by registering and sending my deposit I am acknowledging the above disclaimer. |

| A deposit of $100.00 is necessary to ensure yur reservation. When a trip must be rescheduled, the deposit will be applied to the new trip date. We do not take credit cards so forward the information as noted above and when your date is confirmed as available, you will send your check or money order to the address below. As the time draws near for the trip, we will keep yu posted on weather trends and conditions and either send or e-mail you a local map to our dock as needed.
